New Hire Paperwork

Welcome to The Ohio State University at Newark! 

In order to ensure smooth processing into our Administrative HR System, please complete the following forms in ink and return to the Newark Campus Office of Human Resources, 1179 University Drive, Newark, Ohio 43055.  You will only need to complete the State of Ohio and Federal (W-4) tax card if you have been employed with The Ohio State University during the past 12 months from the effective date of your offer letter and have previously completed all forms.  The Social Security Form SSA-1945 is a new form that became required as of January 1, 2005, which you may not have previously completed. 

If you are working in a benefits-eligible appointment, you will receive a benefits enrollment pack directly from the Columbus campus once your appointment is processed in the HR System.  You will have 30 days from the receipt of the packet to make your benefit selections. 

As an university employee, you share in the responsibility for assuring that the university maintains an environment for work and study free from harassment.  The university seeks to eliminate sexual harassment by encouraging employees to report concerns or complaints and through education.  In accordance, all university employees are required to successfully complete Preventing Sexual Harassment Training.  Please complete this online technology-based training program on Sexual Harassment Prevention in the workplace by following this link:

Please read the document entitled "Best Practices and Procedures for Securing Sensitive Information" carefully. All employees of The Ohio State University at Newark, including employees who are cost-shared with Central Oho Technical College, are required to follow the practices and procedures articulated in this document to protect against the potential loss or theft of sensitive data.

1 In accordance with federal law, The Ohio State University is required to verify the identity and work authorization of all new employees.  As such, two (2) original forms of identification must be provided for verification purposes.  If you are coming from out of the area or cannot stop by the HR Office before your start date, please complete and return all of the other forms and return them to the Office of Human Resources at your earliest convenience and make arrangements to stop by the HR Office upon your arrival on campus to present your two forms of I.D. and complete the I-9.  Please refer to the last page of the “Employment Eligibility Verification” form for the list of acceptable documents.  Due to federal guidelines, I.D.’s that are copied cannot be accepted.

2 In accordance with Ohio Department of Public Safety, Division of Homeland Security, this form serves as a declaration of the provision of material assistance to a terrorist organization or organization that supports terrorism as identified by the U.S. Department of State Terrorist Exclusion List (see the Ohio Homeland Security Division website for a reference copy of the Terrorist Exclusion List).  The Terrorist Exclusion List must be reviewed by new employee prior to completion of form.
